Its four published human trials measured appetite and intake, its effects in 24-hour water-only fasts, and its use as a weight-loss aid in a 24-week study. In healthy men, Amarasate raised GLP-1, CCK, and PYY after a meal and cut calorie intake at the next meal by 18%.14 It significantly reduced hunger during the back half of a 24-hour fast.15 In women, it significantly reduced appetite and food cravings, cutting energy intake at the next meal by 14.3%.16

Recently, the results of a 24-week study comparing Calocurb to placebo in 150 overweight and obese patients have been published.17 They demonstrate that the increases in GLP-1, CCK, and PYY that Amarasate produces translate into real-world weight loss. The Calocurb group lost an average of 4.3% — statistically significantly more than the placebo group.18 They actually lost more fat than weight because they slightly increased their muscle mass, so adjusting for that slight gain in muscle mass meant their overall weight loss was 5.3%.19

Let's compare that to semaglutide. Remember the approximately 15% weight loss achieved in the pivotal semaglutide study? That was a 68-week study in very controlled conditions with a lot of lifestyle interventions (diet and exercise support). In contrast, the recent Calocurb study was over 24 weeks, and the only lifestyle support the participants received was a group counseling session once every 4 weeks from a dietitian. It's therefore fairer to compare the Calocurb study to a study of "real world" use of semaglutide, which showed a 10.9% weight loss after 6 months.20

Side Effects, Safety, and Interactions

Patient-specific (503A) compounding continues under tighter conditions; 503A typically "gets around" the ban on compounding by adding substances such as vitamins that are said to be lacking in that particular patient. It must also be noted that some pharmacies supplying semaglutide via 503A do so without the necessary sterile compounding licenses or been subject to FDA disciplinary actions.30 Larger-scale (503B) compounding is effectively prohibited and the response of FDA to public comment about this is awaited.
